OVERVIEW

We are seeking a dedicated and compassionate Registered Nurse (RN) to join our home care team. We have been in business for over 7 years. Family owned and operated, we serve veteran clients in and around Madison and Buncomb County. The ideal candidate will self-motivated and have a love for healthcare and enjoys interacting with the veterans and their families. Must be willing to travel to client's homes to perform in-home assessments.

GENERAL RESPONSIBILITIES 1

) Review required Client documentation completed by

RN Team:

a. Assessments - every 30, 60, 90 days. b. Care Plans - Immediately following Certification or Re-Certification (or change in client condition). c. Ensure our ADL's are aligned with VA Case-Mix. d. Contact RN's to discuss any missing information, documentation, or inconsistent data. 2) Review required Caregiver Oversight documentation completed by the

RN Team:

a. Annual Skills & Competency Evaluation b. Annual Performance Review c. 90-Day Supervisor Visit d. Contact RN's to discuss any missing information, documentation, or inconsistent data. 3) Review Care Academy Training results to ensure Caregivers are on track with Training requirements. 4) Monitor Care Analytics (AI generated risk results): a. Review and respond to all entries more than 24 hours old. b. Document action taken (no action required; or a phone call to the assigned RN; or a call advising the client to seek medical attention). 5) Make recommendations that will help improve or standardize the process. 6) Respond to calls from clients and/or caregivers when needed. 7) Be familiar with Apps and have the ability to learn and adapt

QUALIFICATIONS 1

) The applicant must have a current North Carolina RN license in good standing with the

NCBON. 2

) Min. 5 years working experience as an RN. At least 2 years of home care or home health experience preferred. 3) Good communication and documentation skills. 4) Reliable vehicle with current car insurance. 5) Must be able to work independently, efficiently, and within scope of practice. 6) Must be self-starter, able to make reasonable independent decisions, and easily adapts to changes in daily schedule.

ADDITIONAL RESPONSIBILITIES

Tasks that come up and are asked to complete that are within scope of practice. Assist Agency Director as needed. The above caregiver descriptions define basic duties involved with each position.
