You will provide clinical oversight, perform resident assessments, coordinate care, and respond to changes in condition while collaborating closely with the interdisciplinary care team. This position is ideal for an RN who thrives in a fast-paced environment, possesses strong critical thinking skills, and has a passion for serving seniors and post-acute residents. What You'll Do Provide direct nursing care and comprehensive resident assessments. Monitor resident conditions and promptly respond to changes in health status. Administer medications, treatments, and injections safely and accurately. Manage residents with complex medical conditions, including rehabilitation, dementia, chronic illness, and post-acute care needs. Collaborate with physicians, therapists, and interdisciplinary team members to develop and implement individualized care plans. Maintain accurate and timely clinical documentation within the electronic health record (EHR). Ensure compliance with all regulatory, HIPAA, and facility standards. Perform nursing procedures including IV therapy, catheter care, wound care, tube feedings, and other skilled nursing interventions. Provide clinical guidance and support to nursing staff to promote quality resident outcomes. Assist with triage and emergency situations while ensuring resident safety and comfort.
