Job Description
LilyCare Hospice is a community-based hospice serving southern New Hampshire out of Bedford. We care for patients where they live — private homes, assisted living, and skilled nursing facilities. We are a small, nurse-driven team, not a national chain. Every Hospice LNA here carries a manageable assignment and knows their patients by name. We are hiring a Hospice LNA to join our Bedford team. Benefits Medical, dental, and vision insurance 401(k) Paid time off and paid holidays Mileage reimbursement for all patient travel Paid continuing education toward your 12 annual NH LNA contact hours Duties and Responsibilities Provide personal care in the patient's home, including bathing, grooming, oral care, dressing, toileting, and repositioning Assist with transfers, mobility, and safe use of lifts and mobility equipment Observe and report changes in condition, skin integrity, intake, and comfort to the assigned RN Case Manager Document each visit accurately and on time in the electronic record Support patients and families with a calm, respectful presence, including during the final days of life Compensation and Schedule $23.00 to $25.00 per hour, based on experience. The pay range listed represents a general guideline and is not a fixed offer.
Location:
Bedford, New Hampshire, with visits throughout the surrounding territory. This Hospice LNA role is a field position, not a facility shift, and requires daily travel between patient homes. Skills and Qualifications Active New Hampshire LNA license in good standing (New Hampshire licenses nursing assistants as LNAs; the CNA credential from another state must be endorsed to NH before hire) Valid driver's license, reliable vehicle, and current auto insurance Ability to work independently in the home setting without on-site supervision Hospice, home health, or long-term care experience preferred Comfort caring for patients at end of life and communicating with their families Apply today to join LilyCare Hospice as a Hospice LNA.
