Job Description
Hospice Registered Nurse Case Manager LilyCare Hospice is a community-based hospice serving patients and families across southern New Hampshire. We are hiring a full-time Hospice Registered Nurse Case Manager to join our clinical team and help shape hospice care delivery from the ground up. Benefits Medical insurance (PPO plan) Company-paid long-term disability coverage Paid time off and holidays Mileage reimbursement at the federal rate Direct access to clinical leadership — no layers between you and decisions Duties and Responsibilities Serve as the Hospice Registered Nurse Case Manager for a caseload of hospice patients in private homes and facilities — complete assessments, individualized plans of care, and routine visits Provide skilled pain and symptom management in coordination with the attending physician and Hospice Medical Director Participate in IDG meetings and coordinate care across the interdisciplinary team, including hospice aides, social work, and chaplaincy Educate and support patients, families, and caregivers, and supervise hospice aide care per the plan of care Complete timely, accurate EMR documentation and participate in the RN Case Manager on-call rotation Compensation and Schedule The Hospice Registered Nurse Case Manager role pays $90,000 - $105,000/year based on experience. Full-time, Monday-Friday day schedule with a rotating on-call assignment. Territory is southern New Hampshire, based out of Bedford, NH.
