Deliver essential care in a Per Diem role performing comprehensive home health risk assessments for Medicare Advantage plan members residing throughout Hillsborough County. This position offers the freedom to create your own schedule, with flexible shifts available Monday through Sunday. As a Nurse Practitioner specializing in home health, you will travel to patients homes in areas such as Manchester, Nashua, Bedford, and nearby communities, conducting each visit in approximately 45 minutes.

Key Responsibilities:

Perform annual health risk assessments and post-discharge evaluations for Medicare Advantage plan members Review and document past and current medical history, medical conditions, and medication use Educate patients on their diagnoses and medications, ensuring understanding and compliance Identify care gaps and recommend ways to resolve them No need to create ongoing treatment plans or write prescriptions

Qualifications:

Active Nurse Practitioner license in New Hampshire (NH) Board Certification through AANP or ANCC Current CPR certification (BLS required) 6 months or more of current Nurse Practitioner experience preferred MSN degree from an accredited nursing school You will earn between $80$90 per evaluation, along with a mileage stipend and compensation for administrative time.
