We are seeking an experienced Nurse Practitioner to perform per diem home health risk assessments for Medicare Advantage plan members throughout Rockingham County. This is an excellent opportunity for practitioners who value autonomy and flexibility, with shifts available in areas such as Derry, Portsmouth, and Salem. Youll conduct focused, comprehensive health visitstypically lasting 45 minutesin the comfort of patients homes.

Key Responsibilities:

Conduct annual and post-discharge home health risk assessments for Medicare Advantage members Complete evaluations of medical and medication history, including prior and current health conditions Provide clear education to patients regarding their diagnoses and medications Identify gaps in care or follow-up and recommend solutions to enhance patient wellness No prescribing or development of comprehensive treatment plans is required

Qualifications:

Valid Nurse Practitioner license in New Hampshire Board Certification through AANP or ANCC Completion of an MSN from an accredited nursing program Current CPR (BLS) certification At least 6 months of experience as a Nurse Practitioner is preferred This per diem opportunity pays $80$90 per evaluation, with an additional mileage stipend and compensation for administrative tasks.
