This position involves conducting comprehensive risk assessments for Medicare Advantage plan members residing throughout Strafford County, NH, including Rochester, Dover, and Somersworth. Each home visit averages approximately 45 minutes and may consist of annual wellness assessments or post-discharge follow-ups.

Key Responsibilities:

Perform thorough in-home risk assessments, focusing on Medicare Advantage members Review patients prior and current medical histories, including all relevant conditions and medications Educate patients about their health status, chronic and acute conditions, and medication regimens Identify gaps in care and provide actionable recommendations to help resolve them Documentation is focused on assessment; you will not develop treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in New Hampshire Current Basic Life Support (BLS) certification Board certification from AANP or ANCC At least six months of recent Nurse Practitioner experience (preferred) MSN degree from an accredited nursing program Professionals in this role are compensated $80-$90 per completed evaluation.
