A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

Monday to Friday, 8:30 AM - 5:00 PM What to Expect as A Clinical Manager Designee The Office-Based RN Designee will oversee a large clinical caseload of approximately 40-50 pediatric and adult clients remotely from our Saddle Brook office. You will partner closely with 2-3 Visiting RN Designees who complete the required in-home supervisory visits. This role serves as the clinical hub for the office, ensuring all documentation, physician orders, authorizations, and clinical communications are completed accurately and within required regulatory timeframes. While primarily office-based, this RN will occasionally assist Clinical Managers with admissions or other field clinical needs as business demands require.Qualifications:
Required:
New Jersey Registered Nurse License in Good Standing 2 years of homecare experience. Experience caring for infants, pediatrics, and adult clients in the home setting. Strong understanding of Medicaid regulations and home care documentation requirements. Proficiency with electronic medical records (EMR) systems and Microsoft Office applications.Preferred:
Experience with pediatric complex care. Experience managing patients with tracheostomies and mechanical ventilators in the home care setting. Prior experience reviewing clinical documentation and coordinating physician orders. Previous leadership, case management, or clinical coordination experience.Our Clinical Manager Designees Enjoy:
