Key Responsibilities:

Perform home health risk assessments for Medicare Advantage members within Taos County Review and evaluate prior and current medical history, conditions, and medication usage Educate patients on their medical conditions and prescribed medications Identify gaps in care and recommend solutions No requirement to develop treatment plans or prescribe medications

Qualifications:

Active and unrestricted Nurse Practitioner license in New Mexico Current BLS (CPR) certification Board Certification through AANP or ANCC A minimum of six months of NP experience preferred Master of Science in Nursing (MSN) from an accredited nursing program As part of this per diem position, you will receive a competitive rate per evaluation, along with a mileage stipend and reimbursement for administrative time.
