Responsibilities:

Conduct in-home risk assessments for Medicare Advantage members, including annual health evaluations and post-discharge visits Review and evaluate patients current and previous medical histories, ongoing conditions, and medication regimens Educate patients on their medical conditions and prescribed medications to promote understanding and compliance Identify gaps in care and recommend actionable steps for resolution Focus is strictly on assessmentno prescribing or development of treatment plans necessary

Qualifications:

Active Nurse Practitioner license in New Mexico Board Certification through AANP or ANCC Current CPR certification (BLS) MSN from an accredited school of nursing At least six months recent experience as an NP (preferred) Your dedication will be rewarded with competitive payranging from $80 to $90 per evaluationplus a mileage stipend and compensation for administrative time.
