Key Responsibilities:

Complete annual home health risk assessments for Medicare Advantage plan members Review and evaluate patients prior and current medical histories, as well as their medication regimens Educate patients about their conditions, medications, and preventive health strategies Identify gaps in care and recommend appropriate next steps for resolution Conduct thorough visits that last approximately 45 minutes eachfocused solely on assessment, with no requirement for creating treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner License in Nevada CPR certification (BLS required) Board Certification through AANP or

ANCC MSN

degree from an accredited nursing program At least 6 months of current experience as an NP (preferred) Benefit from competitive per-evaluation compensation, receiving $80$90 per completed assessment.
