• Coordinates and directs homecare services with physicians, nurses and hospital discharge planning staff to decide patient's appropriateness for homecare.

• $46.72 per hour, depending on experience

Education/Experience Minimum Required:

Graduate of an accredited RN program. Previous experience in a certified home health agency.

License and current registration to practice as a registered Professional nurse in New York State.
