Population Health Quality Nurse (RN) Make a direct impact on patient health while helping drive quality outcomes across our practices. AdvantageCare Physicians is seeking a Population Health Quality Nurse (RN) to partner with providers, care teams, and patients to improve preventive care, close care gaps, and support chronic disease management. In this role, you'll use your clinical expertise and patient engagement skills to improve Medicare Stars, HEDIS, and quality performance outcomes while enhancing the patient experience. What You'll Do Partner with providers and care teams to improve quality outcomes. Identify and close preventive and chronic care gaps through proactive patient outreach. Support care coordination, transitions of care, and population health initiatives. Utilize quality and performance data to improve patient outcomes. Promote an exceptional patient experience through education and follow-up. Required Qualifications NYS Registered Nurse (RN) License. BSN required. Minimum 2 years of clinical nursing experience. Minimum 1 year of ambulatory care experience. Experience in Medicare Advantage, Managed Care, Population Health, Quality Improvement, Care Management, or Value-Based Care. $87,600.00 - $100,500.00 per year
