$1800.00•$2700.00 POSITION SUMMARY•RN Case Manager

POSITION DUTIES

•Facilitates patient care across the continuum of an assigned number of cases by managing, coordinating, facilitating, and monitoring resources to achieve optimal clinical outcomes, utilization of services, and financial goals; Oversees the discharge process for all patients within the unit/core service area; Integral to the team's effort to establish a safe and appropriate discharge plan/plan of care; Possess a working knowledge of the spectrum of hospital and post-discharge services that are available.

MINIMUM REQUIRED QUALIFICATIONS

•NY RN license BLS 2+ years minimum of acute care hospital experience 1.5+ years recent case management experience (within past 2 years) in an acute care facility of similar size and level as this facility Epic experience

•13-14 weeks

•8:00am-4:30pm SYSTEMS•Epic
