Case Manager - Full-Time 1st Shift BrookHaven Retirement Community Brookville, OH Brookhaven Retirement Community Must be a
Registered Nurse Position Summary:
Coordinates a resident's skilled care according to physician orders and ensures communication of skilled care with the interdisciplinary team, resident and family.
Essential Functions:
Because BrookHaven exists to care for our residents, regular attendance and punctuality are essential to provide quality care
Leadership:
Ability to take action and achieve results through others Commitment to BrookHaven mission, values and philosophy and Service Excellence Initiative Must be able to complete performance requirements independently while using good judgment as well as have the ability to harmoniously work with others Must be able to read, write and understand the English language
Position Responsibilities:
Must follow BrookHaven Policies and Procedures Must meet job related competencies as outlined in the Skills Competency Manual
Pre Admission:
Conducts on-site assessments, verifies payer source, completes pre admission screening form, pharmacy and med surge quotes
Admission:
Review and initiate orders, communicate with care community personnel, greet resident and family Coordinate residents MDS's and Care Conferences Participates in the QA process and "Nurse On-call" program On-going communication regarding resident goals, progress and discharge planning Assumes responsibility for maintaining his/her own professional competence Oversees skilled residents care and works closely with the Office Manager Perform other duties as assigned
Knowledge, Skills and Abilities:
Graduation from an accredited school of Nursing. Current license as a Registered Nurse with the State Board of Nursing. CPR certification recommended.
