Overview We are seeking a dedicated and detail-oriented MDS Coordinator Registered Nurse (RN) to join our dynamic healthcare team. In this vital role, you will lead the comprehensive assessment and documentation process for long-term care residents, ensuring compliance with federal and state regulations, including Medicare and CMS standards. Duties Conduct thorough Minimum Data Set (MDS) assessments for long-term care residents, ensuring accuracy and completeness in accordance with clinical documentation standards. Review medical records, progress notes, and clinical data to verify compliance with Medicare regulations and CMS guidelines. Collaborate with interdisciplinary teams—including nursing, therapy, social work, and case management—to gather comprehensive resident information for accurate MDS coding. Ensure timely completion of MDS schedules, including quarterly reviews, annual assessments, and significant change assessments. Utilize EMR/EHR systems such as PCC to document assessments, update medical records, and manage electronic health records efficiently. Apply CPT coding, ICD-9/ICD-10 coding standards, and DRG (Diagnosis-Related Group) classifications to support accurate reimbursement processes. Participate in utilization management activities like discharge planning and case management to optimize patient care pathways and resource utilization. Maintain knowledge of health regulation policies including HIPAA, long-term care regulations, Medicare regulations, and NCQA standards to ensure ongoing compliance. Support clinical documentation improvement initiatives by reviewing medical documentation for accuracy and completeness aligned with healthcare policies. Assist with training staff on documentation standards and regulatory requirements related to MDS processes. Experience Registered Nurse (RN) license with current state licensure required. Proven experience in long-term care facilities such as nursing homes or skilled nursing facilities. Strong understanding of Medicare regulations, CMS guidelines, ICD coding (ICD-9/ICD-10), CPT coding, DRG classifications, and healthcare reimbursement processes. Familiarity with electronic health record (EHR) systems such as PCC. Experience with clinical documentation standards, medical records review, utilization review, case management, and health information management. Ability to navigate state healthcare regulations including HIPAA compliance and long-term care policies effectively. Strong physiology knowledge along with medical terminology expertise to accurately interpret clinical data. Excellent communication skills to collaborate across multidisciplinary teams.
