00 + Trip Fees Cornerstone Home Health, Inc. is looking for an experienced Registered Nurse to provide skilled nursing services to patients throughout the Oklahoma City metro, surrounding communities, and the Weatherford area. This is a per-visit position with flexible scheduling and is ideal for an RN who is comfortable working independently in the home health environment, managing a changing visit schedule, and completing clinical documentation within established time frames. Previous experience completing skilled nursing visits and OASIS documentation is required. Candidates must have experience with Start of Care, Resumption of Care, Recertification, Transfer, and Discharge visits. Home health experience is preferred. Compensation & Position Details This is a flexible, per-visit position . $45.00

00 per visit , based on experience and type of visit Trip fees provided Flexible scheduling Weekday workweek Limited weekend needs may occur under special circumstances Nurses must have availability beginning as early as 8:00 a.m. and extending through 6:00 p.m. Participation in required Case Conference meetings is also part of this position. Your Role As a Home Health RN with Cornerstone, you will independently manage assigned patients while working closely with physicians, therapists, and other members of the interdisciplinary care team.
Your responsibilities will include:
Performing comprehensive nursing assessments in patients' homes Evaluating vital signs, medical history, current condition, and ongoing clinical needs Developing and updating individualized plans of care based on assessments and physician orders Performing skilled nursing visits and coordinating post-acute care Completing required OASIS assessments and documentation for Start of Care (SOC), Resumption of Care (ROC), Recertification, Transfer, and Discharge Monitoring patients for changes in condition, complications, or emerging clinical concerns Communicating significant findings and coordinating appropriate interventions with physicians and other healthcare professionals Educating patients and families regarding medications, disease management, treatment plans, and ongoing health maintenance Providing wound care, IV therapy, medication management, and other skilled nursing services when appropriate Maintaining complete, accurate, and timely clinical documentation Following HIPAA requirements, agency policies, and applicable standards of care Using independent clinical judgment to recognize and appropriately respond to urgent patient needs Required Qualifications Applicants must meet the following requirements: Current and valid Registered Nurse (RN) license in Oklahoma Proven experience performing skilled nursing visits Experience completing OASIS documentation Experience with SOC, ROC, Recertification, Transfer, and Discharge visits Strong patient assessment and clinical decision-making skills Working knowledge of medical terminology and physiology Ability to independently organize patient visits, prioritize assignments, and meet documentation deadlines Strong verbal and written communication skills Ability to effectively collaborate with patients, families, physicians, and interdisciplinary healthcare professionals Preferred Clinical Experience Additional experience in any of the following areas is beneficial: Home health Wound care IV therapy Medication management Critical care or ICU Emergency department Hospital medicine Post-acute care Geriatrics Infectious disease care Behavioral health Travel & Service Area This position involves traveling directly to patients' homes throughout the Oklahoma City metro and surrounding areas . The RN will also travel to Weatherford, Oklahoma approximately a couple of times per month . Applicants should be comfortable traveling between patient locations as part of their regular assignments.
