RN Navigator - Mercy Mercy, a leading healthcare organization in the Midwest, seeks an RN Navigator to coordinate and personalize care for patients across the healthcare continuum. In this Healthcare & Medical Services role, you will guide patients through complex care pathways, facilitate timely referrals, and collaborate closely with interdisciplinary teams to reduce barriers and improve outcomes. You'll educate patients and families, support discharge planning, and leverage EHR tools to track progress and quality metrics. Responsibilities Serve as primary RN Navigator for assigned patient population, guiding patients across the care continuum. Coordinate appointments, referrals, and follow-up care to ensure timely, seamless access to services. Perform nursing assessments and develop individualized care plans in collaboration with providers. Educate patients and families on diagnoses, treatment options, medications, and self-management. Collaborate with physicians, social workers, and interdisciplinary teams to remove barriers to care. Monitor patient progress, identify high-risk patients, and intervene to prevent avoidable readmissions. Advocate for patients, connecting them with community resources, financial counseling, and support services. Document all navigation activities in the EHR and maintain accurate, timely clinical records. Use data and metrics to track outcomes, report trends, and support quality improvement initiatives. Participate in staff education, program development, and best-practice implementation within Mercy.
