Key Responsibilities:

Complete comprehensive annual home health risk assessments for health plan members Review and evaluate prior and current medical history, medical conditions, and medication history Educate patients on their conditions and medications to empower better outcomes Identify potential gaps in care, recommending necessary next steps to optimize health Focus strictly on evaluationno need to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in Oregon Current CPR (BLS) certification Board certification through AANP or
