Key Responsibilities:

Conduct annual and post-discharge home health risk assessments for plan members Evaluate patients current and prior medical history and medication usage Educate patients about their diagnoses and all prescribed medications Identify gaps in care and suggest practical recommendations No requirement to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in Oregon Board certification through AANP or ANCC Current CPR certification (BLS) MSN from an accredited nursing school Minimum of 6 months current experience as a Nurse Practitioner preferred
