A unique per diem opportunity is available for an experienced Nurse Practitioner to perform home health risk assessments. These visits typically last about 45 minutes and are conducted in various locations throughout Polk County, including Dallas, Salem, and Falls City. The role involves working with Medicare Advantage plan members and tailoring your schedule to suit your availability.

Schedule:

Flexible shifts available Monday through Sunday Create your own schedule with minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

Conduct annual home health risk assessments in member residences Review past and present medical history, current health status, and medication use Offer patient education regarding diagnoses and medication management Identify care gaps and present recommendations to close those gaps No requirement to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in Oregon Board certification (AANP or ANCC) Current BLS (CPR) certification Minimum 6 months of recent Nurse Practitioner experience is preferred MSN degree from an accredited school of nursing This role provides compensation of $80-$90 per evaluation, in addition to a mileage stipend and pay for administrative time.
