Key Responsibilities:

Perform annual health and post-discharge risk assessments in patients homes Review and evaluate medical history, current conditions, and medication regimens Provide patient education regarding health conditions, medications, and preventive measures Identify gaps in care and make recommendations to enhance patient outcomes No treatment plan formulation or prescribing requiredfocus is on assessment and education

Qualifications:

Active Nurse Practitioner license in Oregon Board Certified through AANP or ANCC Current CPR certification (BLS) MSN degree from an accredited nursing institution At least 6 months of recent experience as an NP preferred Professionals in this role receive $80$90 per completed evaluation, with a mileage stipend and compensation for administrative time.
