Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage members Thoroughly review patient medical histories, current conditions, and medication use Educate patients on medical conditions and appropriate medication management Identify gaps in care and recommend actionable solutions Visits may include annual health assessments or post-discharge follow-up No need to develop treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in Oregon Board Certification (AANP or ANCC) MSN from an accredited nursing program Current CPR/BLS certification At least six months of nurse practitioner experience preferred As part of this role, you will receive $80-$90 per completed evaluation, along with a mileage stipend and compensation for administrative time.
