Position Summary:
We are seeking an experienced Registered Nurse (RN)
  • Adult Home Health to provide high-quality, patient-centered care to adult patients in their homes.
The ideal candidate must have at least 2 years of recent home health nursing experience and strong knowledge of OASIS assessments and documentation .

Card Roles & Responsibilities:
Perform comprehensive nursing assessments and provide skilled care to adult patients in their homes. Complete accurate OASIS assessments and maintain timely, compliant clinical documentation. Develop and implement individualized care plans based on each patient's clinical needs. Administer medications, perform treatments, monitor patient conditions, and communicate changes to physicians. Educate patients and caregivers on disease management, medications, safety, and home care plans.
Skills:
Strong adult home health nursing and patient assessment skills. Proficient in OASIS assessments, documentation, and care planning . Skilled in medication administration, wound care, and clinical treatments. Excellent patient education, communication, and care coordination skills. Strong clinical judgment, time management, and ability to work independently. RN01
