Primary Responsibilities:

Conduct comprehensive home health risk assessments, including annual health reviews and post-discharge visits Gather and assess prior/current medical history, existing health conditions, and medication usage Provide educational support on diagnosed conditions and prescribed medications Recognize and report any gaps in care, sharing actionable recommendations to promote positive health outcomes

You will not be responsible for creating treatment plans or prescribing medications

Required Qualifications:

Active Nurse Practitioner license in Oregon Board Certification through AANP or

ANCC MSN

degree from an accredited nursing program Current CPR certification (BLS) At least six months of recent Nurse Practitioner experience is highly preferred
