Responsibilities include:

Conducting annual risk assessments for Medicare Advantage plan members in their homes Completing evaluations of past and current medical history, medical conditions, and medications Educating patients on their medical conditions and prescribed medications Identifying gaps in care and making recommendations for closing these gaps Engaging members through annual health assessments or post-discharge visits No requirement for creating treatment plans or prescribing medications

Qualifications:

Valid Nurse Practitioner license in Oregon Current CPR certification (BLS) Board Certification through AANP or

ANCC MSN
