An opportunity is available for an experienced Nurse Practitioner to provide home health risk assessments for Medicare Advantage plan members. This per diem role offers the freedom to tailor your schedule and impact patients in Salem and throughout Marion County, including Keizer, Woodburn, and Hayesville. The primary focus will be on conducting thorough, in-home evaluationsgenerally lasting about 45 minutes per visitwhich may take the form of annual health assessments or follow-up, post-discharge check-ins.

Key Responsibilities:

Conduct annual in-home health risk assessments for Medicare Advantage beneficiaries Evaluate current and prior medical history, active medical conditions, and medication regimens Educate patients about their medical conditions and medication management Identify gaps in care, providing recommendations for follow-up or additional care as needed No requirement to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in Oregon Current Basic Life Support (BLS) certification Board Certified through AANP or ANCC Minimum of six months of recent nurse practitioner experience preferred MSN from an accredited nursing program This position offers compensation of $80-90 per evaluation, in addition to a mileage stipend and paid administrative time, making it both rewarding and flexible.
