Key Responsibilities:

Perform comprehensive, in-home annual health risk assessments for Medicare Advantage plan members residing in areas including Salem, Keizer, Woodburn, and Hayesville Evaluate patients medical and medication histories, including both current and prior conditions Educate patients regarding diagnoses, medications, and best practices for maintaining their health Identify any gaps in care and provide evidence-based recommendations to improve health outcomes Visits may include annual assessments or post-discharge check-ins, tailored to the patients needs Role does not include developing treatment plans or prescribing medications

Qualifications:

Valid Nurse Practitioner license in the State of Oregon Active CPR certification (BLS) Board Certification through AANP or ANCC Minimum of 6 months current Nurse Practitioner experience preferred Master of Science in Nursing (MSN) from an accredited institution Enjoy compensation of $80-$90 per completed evaluation, in addition to a mileage stipend and administrative time.
