Seeking a knowledgeable Nurse Practitioner experienced in home health to conduct comprehensive risk assessments for Medicare Advantage plan members across Polk County, OR, including areas such as Dallas, Salem, and Falls City. This per diem position is ideal for professionals who value flexibility and autonomy, offering the ability to create your own schedule and support members during in-home visits.

Schedule:

Flexible shifts available Monday through Sunday Self-scheduling to fit your availability Commitment options on a weekly or bi-weekly basis

What You'll Do:

Perform annual risk assessments in members' homes, each typically lasting about 45 minutes Evaluate medical history, current conditions, and medication use Educate patients regarding their health conditions and medication regimens Identify care gaps and recommend solutions to improve health outcomes

This role does not include developing treatment plans or prescribing medications

Preferred Skills & Background:

Active Nurse Practitioner license in Oregon Current CPR certification (BLS) Board Certification from AANP or ANCC Master's degree in Nursing (MSN) from an accredited institution At least 6 months of recent experience practicing as a Nurse Practitioner preferred
