Key Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage plan participants in various locations within Marion County Review and evaluate patients medical histories, ongoing conditions, and medication regimens Educate patients about their health conditions and associated medications Identify care gaps and make recommendations to facilitate optimal health outcomes No requirement to develop treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in the state of Oregon Current CPR (BLS) certification Board Certification through AANP or ANCC At least 6 months of recent experience as a Nurse Practitioner preferred Master of Science in Nursing (MSN) from an accredited institution This per diem role offers $80$90 per evaluation, with additional mileage reimbursement and compensation for administrative time.
