Key Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage members, typically within a 45-minute appointment window Evaluate members medical histories, current conditions, and medication profiles Educate individuals on their medical conditions and medication management Identify gaps in care, offering informed recommendations to promote health outcomes Complete risk assessments during various visit types, such as annual health assessments or post-discharge check-ins Prescribing medications and developing treatment plans are not required

Qualifications:

Active Nurse Practitioner license in Pennsylvania Current CPR certification (BLS required) Board Certification through AANP or ANCC Minimum of 6 months of current Nurse Practitioner experience preferred Master of Science in Nursing (MSN) from an accredited institution This position provides compensation of $80-$90 per evaluation, with additional mileage stipends and administrative time included.
