Key Responsibilities:

Perform comprehensive annual home health risk assessments for Medicare Advantage plan participants at their residences, with each visit averaging roughly 45 minutes Review and assess both prior and current medical histories, medical conditions, and medication usage Educate patients on their diagnoses and medications, enhancing understanding and compliance Identify any gaps in patient care and provide actionable recommendations to address them No treatment plan development or medication prescribing required

Qualifications:

Active Nurse Practitioner license in Pennsylvania Current CPR certification (BLS) Board Certification via AANP or ANCC required Master of Science in Nursing (MSN) from an accredited institution At least 6 months of experience as a NP preferred Nurse Practitioners in this per diem position receive $80-$90 per evaluation, along with a mileage stipend and compensation for administrative time.
