A skilled Nurse Practitioner is needed to perform home health risk assessments for Medicare Advantage health plan members in Scranton and surrounding areas within Lackawanna County, PA. Key Responsibilities:

Conduct comprehensive annual home health risk assessments during 45-minute home visits Evaluate patients prior and current medical history, medical conditions, and medication use Educate patients on their medical conditions and medication regimens Identify gaps in care and offer clear recommendations to address these issues Visits may include annual health assessments or post-discharge follow-ups No requirement for creating treatment plans or prescribing medications

Qualifications:
