Experienced Nurse Practitioners are sought for per diem home health risk assessments within Lackawanna County, PA. This position offers exceptional schedule flexibility, allowing you to create your own shifts between Monday and Sunday, with only a minimal weekly or bi-weekly hourly commitment required. Assignments may include locations such as Scranton, Carbondale, and Dunmore, providing an opportunity to work closely with Medicare Advantage plan members throughout the county.

Key Responsibilities:

Conduct comprehensive annual health risk assessments in-home for Medicare Advantage members Assess patients prior/current medical history, conditions, and medication regimen Provide education to patients regarding their conditions and prescribed medications Identify and communicate any gaps in care, making recommendations to address these gaps Visits may include annual assessments or post-discharge follow-up No prescribing or treatment plan creation required

Qualifications:

Active Nurse Practitioner License in Pennsylvania Current CPR certification (BLS) Board certification (AANP or ANCC) MSN from an accredited nursing program Minimum of 6 months of current NP experience (preferred) This per diem position provides compensation of $80$90 per completed evaluation, in addition to a mileage stipend and compensation for administrative time.
