  • NEPA, a Modivcare Home Care company, is hiring a Full-Time RN Case Manager to provide one-on-one pediatric home health visits across: Lackawanna, Susquehanna Counties This is a field-based, salaried role with a company vehicle provided.
If you are a pediatric-focused RN who values independence, clinical leadership, and meaningful patient relationships — this role offers the opportunity to lead care in the home setting. Schedule & Travel Expectations Full-Time | 40 hours per week 5 days per week | Monday-Sunday | 40 hrs a week (Flexible schedule) Daily travel across assigned coverage counties based on patient needs This role requires flexibility and comfort with travel within the listed counties. Compensation & Benefits $70,000
