Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately, including IV infusions and catheterizations, following strict aseptic techniques and safety protocols. Assist with diagnostic evaluations, specimen collection/processing, and wound care procedures like suturing and dressing changes. Support patient mobility and comfort through activities such as tube feeding, ventilator management, and Hoyer lift operation. Collaborate with multidisciplinary teams in acute care settings like ICU, emergency room, or surgical units to coordinate patient care plans. Maintain clinical confidentiality policies while documenting medical records thoroughly and accurately in EMR/EHR systems. Provide specialized care for patients with complex needs including dementia, Alzheimer's disease, infectious diseases, or those requiring hospice or palliative care. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN). Proven experience working with geriatric patients in long-term care facilities, skilled nursing facilities, or assisted living environments. Demonstrated proficiency in EMR/EHR systems such as Epic, Cerner, or Athenahealth; familiarity with

ICD-9/ICD-10

coding is preferred. Critical care experience in settings like ICU or trauma centers is highly desirable. Strong knowledge of anatomy, physiology, pharmacology, and medical terminology relevant to diverse patient populations. Ability to perform physical examinations, venipuncture (phlebotomy), injection administration, and sterile procedures confidently. Excellent communication skills for triage assessments, patient education, and interdisciplinary collaboration.
