Key Responsibilities:

Conduct annual health risk assessments in members homes, with each visit averaging 45 minutes Review and evaluate medical histories, current health conditions, and medication usage Provide clear patient education on chronic conditions and medication management Assess gaps in care and provide actionable nursing recommendations Visits may also include post-discharge assessments as needed No requirement to develop treatment plans or prescribe medications

Qualifications:

Current Nurse Practitioner license in Rhode Island Board certification by AANP or ANCC Completion of an accredited MSN program Active CPR certification (BLS) Six months or more of recent NP experience preferred
