Advanced practice clinicians skilled in home health are needed for per diem opportunities performing comprehensive risk assessments. This role involves traveling to patients residences within Florence County, SCincluding Florence, Lake City, and Johnsonvilleto support a Medicare Advantage health plan.

Key Responsibilities:

Conduct home health risk assessments, each averaging 45 minutes per visit Review and evaluate prior and current medical history, chronic conditions, and medications Educate members on disease processes, preventive care, and proper medication use Identify and highlight any care gaps, offering recommendations for follow up No ongoing treatment planning or medication prescribing is required

Desired Qualifications:

Active Nurse Practitioner license in South Carolina Board Certification by AANP or ANCC Current CPR (BLS) certification MSN degree from an accredited nursing program Minimum six months experience as a practicing Nurse Practitioner is preferred
