Advance your career as a Nurse Practitioner by delivering essential home health risk assessments across Chesterfield County, SC. In this per diem opportunity, you will serve Medicare Advantage members in a home health capacity, with assessments typically lasting about 45 minutes. Assignments may include annual health assessments or post-discharge visits, based on patient needs. Flexible shifts are available Monday through Sunday, allowing you to set your own schedule and define your weekly or bi-weekly commitment. You may be scheduled for visits in Cheraw, Chesterfield, Pageland, or nearby areas within the county.

Key Responsibilities:

Perform comprehensive in-home health risk assessments for Medicare Advantage plan members Review and evaluate prior and current medical history, chronic and acute conditions, and medication management Educate patients regarding existing medical conditions and medication usage Identify care gaps and recommend appropriate measures to address them

You will not be required to create treatment plans or prescribe medications

Qualifications:

Active and unrestricted Nurse Practitioner (NP) license in South Carolina Valid CPR (BLS) certification National board certification (AANP or ANCC) MSN degree from an accredited nursing school At least 6 months of recent NP experience preferred This position offers competitive compensation of $80-$90 per evaluation, plus a stipend for mileage and administrative time.
