This position involves conducting home health risk assessments for Medicare Advantage plan members across Green County, including Monroe, Brodhead, and New Glarus. Your Key Responsibilities:

Complete annual home health risk assessments in members' homes Conduct thorough evaluations of prior and current medical history, medical conditions, and prescriptions Offer education regarding diagnoses and medications to empower patients Identify and report gaps in care, suggesting appropriate recommendations No responsibility for treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in Wisconsin Board Certification from the AANP or ANCC Current BLS/CPR certification Master of Science in Nursing (MSN) from an accredited institution Six months of recent NP experience preferred Enjoy the competitive rate of $80-$90 per evaluation, plus a mileage stipend and compensation for administrative time.
