Key Responsibilities:

Conduct annual home health risk assessments for Medicare Advantage members Review prior and current medical history, medical conditions, and medication use Educate members regarding their medical conditions and medications Identify and address gaps in care, offering recommendations for improvement Document findings accurately and efficiently

Qualifications:
