Key Responsibilities:

Perform comprehensive annual and post-discharge home health risk assessments for plan members in locations such as Oak Hill, Fayetteville, and Montgomery Review and evaluate medical history, existing conditions, and current medication regimens Educate patients on their medical conditions and prescribed treatments to foster informed decision-making Identify potential care gaps and suggest actions to address them No ongoing treatment planning or prescribing requiredfocus on thorough and accurate assessment

Qualifications:

Active Nurse Practitioner license in West Virginia Current BLS (CPR) certification Board Certification through AANP or ANCC Master of Science in Nursing from an accredited program At least 6 months of recent NP experience is preferred
