Key Responsibilities:

Perform comprehensive home health risk assessments for Medicare Advantage members Complete clinical evaluations averaging 45 minutes per visit, focusing on annual health assessments or post-discharge needs Review patients prior and current medical history, medical conditions, and medication regimens Educate patients about their health conditions and medications Identify any gaps in care and provide recommendations to help close those gaps No responsibility for prescribing medications or creating treatment plans

Desired Qualifications:

Current Nurse Practitioner license in West Virginia Basic Life Support (CPR) certification Board Certification through AANP or

ANCC MSN
