Each assessment averages about 45 minutes, and visits may range from annual evaluations to post-discharge follow-ups.

Key Responsibilities:

Perform comprehensive annual home health risk assessments for Medicare Advantage members Assess both current and prior medical history as well as medication usage Educate patients on their conditions and medication regimens Detect and address care gaps, offering actionable recommendations No requirements for writing treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner licensure in West Virginia Board Certification via AANP or ANCC Current CPR (BLS) certification MSN degree from an accredited nursing program A minimum of 6 months' experience as a Nurse Practitioner is preferred This position offers compensation of $80-$90 per completed assessment, along with a mileage stipend and compensation for administrative duties.
