Schedule:

13-week contract with possible extension Full-time, Monday through Friday On-call responsibilities may be required Daily visit expectations

Key Responsibilities:

Deliver high-quality in-home nursing care and interventions Conduct thorough patient assessments and document OASIS data accurately and on time Work collaboratively with physicians and medical team members to implement individualized care plans Complete wound care, manage medications, educate patients, and monitor chronic conditions Communicate promptly about changes in patient status to the clinical team

Qualifications:

Active RN license At least 1 year of recent RN experience in home health Proficient with OASIS documentation (required) Experience using HomeCare HomeBase (strongly preferred) Proven ability to manage a set, full-time visit schedule Reliable transportation and a valid drivers license
