This vital role involves engaging with community members, assessing individual needs, and guiding clients through the admissions process to ensure they receive the appropriate care and support. Duties Conduct outreach activities within the community to identify individuals who may benefit from transitional housing and IOP services. Perform comprehensive intake assessments, including gathering client histories, evaluating needs, and documenting information accurately in electronic health records (EHR). Coordinate with clinical teams to facilitate smooth admissions, discharge planning, and ongoing case management for clients. Provide education about program offerings, eligibility requirements, and community resources available for individuals with substance use disorders or mental health conditions. Maintain detailed documentation of client interactions, assessments, and referrals while ensuring compliance with HIPAA regulations. Collaborate with community organizations, healthcare providers, and social service agencies to expand outreach efforts and resource availability. Prioritize tasks effectively to manage a high volume of client inquiries while delivering exceptional patient service and support. Skills Experience working with individuals with substance use disorders, mental health conditions, or disabilities in outpatient or inpatient settings. Strong knowledge of medical terminology, physiology, diagnostic evaluation, and clinical case management principles. Familiarity with hospital or healthcare environments such as Level I Trauma Centers or Level II trauma centers is preferred. Proficiency in electronic health records (EHR) management systems and understanding of HIPAA compliance standards. Excellent client communication skills with the ability to work empathetically with diverse populations including children and families. Ability to prioritize tasks efficiently in a fast-paced environment while maintaining attention to detail. Background in social work, behavioral health, or addiction counseling is highly desirable. Knowledge of community resource coordination in health services social work enhances effectiveness in this role.
