IV League, Inc. is a home infusion pharmacy coordinating the transition of hospitalized patients to their home setting. A Patient Care Coordinator is responsible for arranging all home infusion services needed for patients. We are looking for a dynamic, multi-tasking coordinator to become a part of our Intake team. Patient Care Coordinator responsibilities include the following: Receive healthcare referrals from various healthcare organizations via phone/fax/online. Ensure that all referrals are keyed into EMR system. Complete patient admin packets for all new patients by communicating with case managers, home health agencies, physicians, payers, patients and caregivers. Document communication effectively and efficiently. Run BI (Benefits Investigations): insurance verification, eligibility determination and authorizations via phone or web portals. Act as liaison between the patient, home health agency, and physician for care coordination. Ensure the patient's and referral source's needs are met timely and accurately. Collaborate closely with Clinical Liaison team to improve patient outcomes.
Minimum Qualifications:
Must possess excellent oral and written communication skills, with the ability to express technical issues in simple terms. Experience:
Must have a high school diploma or GED or higher. Pharmacy Technician or Billing and Auth experience is preferred. Prior experience in a pharmacy or home health company is preferred.
