Job Description
Job Overview We are seeking a highly motivated and experienced Intake Coordinator who is fluent in Spanish and has experience in coordination of care in the medical field. This role involves managing the initial contact and assessment process for clients. The ideal candidate will possess strong communication skills, cultural competence, and experience in social services or behavioral health settings. Your energetic approach will help connect individuals with the resources they need, ensuring a smooth intake process that promotes positive outcomes and supports high-risk populations. Duties Conduct bilingual (Spanish-English) client intake interviews with empathy, professionalism, and cultural sensitivity Gather comprehensive client information through detailed data entry into our case management systems Maintain accurate documentation of client interactions, service plans, and progress notes in compliance with confidentiality standards Assist with case management and providing client communication tailored to individual needs Skills Exceptional organizational skills for data entry, case management documentation, and client tracking Fluency in Spanish and English with excellent verbal and written communication skills Must be someone who is capable, smart, organized and effecient Proficiency in Google Suites (Word, Excel, Outlook) for documentation and reporting purposes Ability to handle crisis situations effectively through crisis management skills and high-risk population support techniques Experience working with individuals with disabilities or special needs, demonstrating compassion and adaptability Demonstrated ability to work independently while collaborating within a multidisciplinary team environment Writing skills capable of producing clear reports, client summaries, and service plans that meet professional standards Join us in making a difference by providing compassionate intake coordination that empowers individuals to access vital services.
