Job Overview We are seeking a proactive and empathetic Clinical Intake Coordinator to join our dedicated team. In this vital role, you will serve as the first point of contact for patients and families, coordinating intake assessments, gathering essential medical and psychosocial information, and ensuring seamless communication between clinical teams and community resources. Your efforts will help us deliver exceptional care from the very first interaction. Responsibilities Conduct comprehensive patient intake assessments, including medical history, psychosocial factors, and current needs Coordinate with healthcare providers, social workers, and community agencies to facilitate timely access to services Manage electronic health records (EHR) with accuracy while ensuring compliance with HIPAA regulations Communicate clearly with patients and families to explain services, gather necessary documentation, and answer questions Prioritize tasks effectively to handle multiple intakes efficiently while maintaining attention to detail Assist in discharge planning by gathering relevant information for ongoing care needs Collaborate with clinical teams to support patient assessment processes and ensure continuity of care Requirements Previous health care experience Knowledge of medical terminology, physiology, and diagnostic evaluation methods Familiarity with electronic health records (EHR) systems and healthcare compliance standards like HIPAA Strong communication skills for working with diverse patient populations, including children and individuals with disabilities or mental health conditions Ability to prioritize tasks efficiently in a fast-paced environment while maintaining accuracy Basic life support (BLS) certification Compassionate approach to patient service and case management; experience in social work or behavioral health is advantageous Join us at Solterra Hospice where your dedication will directly impact lives through compassionate care. Pay:
$26.00 - $30.00 per hour Expected hours: 40.0 per week
