Job Description About the Opportunity PRINE Health is looking for an Enrollment Specialist to join our growing Kidney Health Initiatives team. This role is best suited to someone with a sales mindset who thrives on outreach, persuasion, and closing — paired with genuine comfort discussing health topics with patients. Position Summary The Enrollment Specialist is responsible for driving patient participation in PRINE Health's Kidney Health Initiatives and related care management programs, with a primary focus on educating patients and enrolling them into Chronic Care Management (CCM) and other applicable programs. This is fundamentally a sales-driven role: you'll manage a high-volume pipeline of prospective patients, work through objections, and convert eligible patients into enrolled program participants — all while representing PRINE Health's clinical programs accurately and building genuine patient trust. Some direct CCM care coordination duties are included, but the core function is outreach, persuasion, and enrollment. Key Responsibilities Patient Outreach, Sales & Enrollment Contact eligible patients (via phone, in-office, and other outreach channels) to explain Kidney Health Initiative and CCM program offerings and sell patients on the value of enrolling Handle objections and hesitations skillfully, addressing misconceptions and closing enrollment where clinically appropriate Guide patients through the enrollment process, including consent, intake documentation, and initial program setup Maintain and work an active enrollment pipeline, prioritizing outreach based on clinical urgency, enrollment deadlines, and conversion likelihood Track outreach activity, conversion outcomes, and follow-up needs in the EHR/CRM system, similar to a sales CRM workflow Program Support (CCM) Support light CCM care coordination tasks as needed (e.g., care plan check-ins, scheduling coordination, documentation of monthly time logs) Serve as a point of contact for enrolled patients with basic program questions, escalating clinical concerns to appropriate clinical staff Ensure enrolled patients' records reflect accurate program status and required documentation for compliance/billing purposes Independent Case Management & Performance Manage own daily/weekly outreach schedule and enrollment targets without close supervision Identify patterns in declines or drop-off and adjust sales/outreach approach accordingly to improve conversion rates Maintain organized, up-to-date records for every patient contact and enrollment status Report enrollment metrics, conversion rates, and program progress to management on a regular cadence Qualifications High school diploma required; Associate's or Bachelor's degree in healthcare administration, health sciences, business, or a related field preferred 1-3 years of experience in a sales, telesales, patient outreach, healthcare call center, or enrollment/conversion role (nephrology or chronic disease management experience a plus) Demonstrated ability to persuade, handle objections, and close — proven sales or conversion track record preferred Strong verbal communication skills, comfortable having in-depth, sometimes sensitive conversations with patients about their health and program options Highly organized and self-directed; able to manage a full pipeline/caseload independently and hit enrollment targets Comfortable with EHR/CRM systems and basic documentation/reporting tools Knowledge of CCM billing/documentation requirements a plus, but not required at hire Bilingual (Spanish or other languages relevant to patient population) a plus
